Request for Waiver of Standard
Fee Structure
In some circumstances a waiver of a portion
of the fee is appropriate. If you feel that your
situation warrants such consideration please
submit the following request.
Must meet all Criteria:
1. Completed "Book Doc" form (with
all sub fields completed)
2. Flexibility in planning and scheduling of
your event.
3. All travel expenses covered in addition to
speaking fees.
4. Any school, conference, district, grant program, or corporation that contractually agrees to purchase the required minimum product package (any combination of DOCSPEAKS teacher books, student guides, DVD's, CD's, and/or prevention posters) that reinforce the content of Doc's message, is eligible for a fee waiver.
Or
5. Booking Doc while he is
in your area on an already scheduled event. (See
Doc's Calendar). Please indicate
the Name and Date of the scheduled event.
Please submit your detailed requests for a waiver
from the standard fee structure. Please explain
fully what financial restrictions and/or limitations
you feel justify a waiver. Please limit your
response to 500 words or less. Please be advised
that the submission of this request does not
guarantee the granting of a waiver.
